Elevate Physical Medicine & Rehabilitation is an out-of-network provider. As such, you will be billed directly by us for your services. Upon request, we will provide you with a fully coded insurance-ready receipt (a “superbill”) to submit to your insurance company for potential reimbursement.
While in-network insurance providers typically see a high volume of patients, our out-of-network model allows us to offer a higher level of personalized care. At your first visit, you can expect up to 60-90 minutes of one-on-one time with Dr. Panjwani, giving us the opportunity to thoroughly assess your needs, perform a detailed evaluation, and create a customized treatment plan focused on addressing the root causes of your concerns.
